New Patient Forms
We understand that along with quality dentistry, our patients expect excellent services and convenient, timely care. To that end, we’ve provided our patient forms online. We encourage you to print and fill them out at your convenience, then bring them with you to your first appointment. This will help us make the check in process smooth and keep your appointment right on schedule.
If you prefer to prepare your paperwork in our office, that’s fine too. Just be sure to arrive about 15 minutes early so that we can keep your visit on time.
Payment Options
Vacek Family Dentistry is a fee for service provider. However, we are happy to assist you in filing a claim with your insurance provider in order to maximize benefits.
We accept cash, check, and all major credit cards, including Visa, MasterCard, Discover, and American Express. If you prefer to finance your dental work, we’ve teamed with two trusted sources in medical financing, CareCredit and CitiHealth Card. You may visit them online for more information or talk with us about them at your next office visit.
